Job Summary

 

Within the framework of the objectives and policies established by the Company, plans and coordinates clinical services with those of the administrative systems. Participate as a member of the company’s management team in planning, policy formulation, and administrative decision making with particular reference to the role, functions, and operations of the agency’s clinical services. In summary, is responsible for patient care, clinical, resource management, and oversight of the Quality Improvement Program alongside the Director of Nursing.

 

The Registered Nurse role must have healthcare management experience and home healthcare / home care / hospital work experience with multiple age groups. Someone with working knowledge of local, state, and federal funding sources and other applicable regulations would be a good fit for this dynamic leadership role.

  • Assist in supervision of all licensed staff (LPN, RN, and other future licensed staff). Directs, coordinates and supervises all clinical functions as well as maintains compliance with federal, state and local law and regulations as they apply to the provision of home visits to clients.
  • Assist in administering nursing program in home care to maintain standards of patient care, and also advises medical staff, department heads, and also administrators in matters related to nursing services. Assures compliance with agency policy and procedure,  monitors job performance and completion of evaluations and counseling of clinical staff.  This will be documented through:
  1. Conducting and evaluation of clinical performances of all RN’s and LPN’s staff annually, or more frequently if need is indicated.
  2. Plans and implements in-service and continuing education programs to meet education and training needs of the organizations Clinical personnel.
  • Assists in directing, coordinating and supervising clinical functions as well as maintains compliance with federal, state and local law and regulations as they apply to the provision of home visits to clients.
  • Assures compliance with agency policy and procedure,  monitors job performance and completion of evaluations and counseling of clinical staff
  • Initiate non-medical and skilled nursing services pricing and billing through collaboration with the office and DON.. Annually update pricing of services, communicate to clients and update individual service contracts as necessary.
  • Assists in recruitment, hiring, supervising, training, performance management and corrective action of clinical staff.
  • Assists in monitoring clients for changes in condition, medication & nutrition needs and update care plan, physician and family as necessary. Provide transitioning client with emotional support and encouragement towards activity participation.
  • Assists in review chart documentation and service agreements to ensure compliance with applicable federal, state and local laws, rules, and regulations; maintains knowledge of and follows all company policies and procedures. Policy development and monitoring must be in-line with the Joint Commission standards, as needed.
  • Team member engagement, relations, recognition and retention will be recorded by the DON and the Registered Nurse to determine trends of personnel and adjusted through collaboration with the president of the company.
  • Ensures client satisfaction levels are achieved through quality initiatives.
  • Review client referral information and respond to home care service requests and inquiries in a timely manner as directed by the DON
  • Assists in demonstration of fiscal accountability by monitoring operations to ensure budget preparation, cost control, and inventory match the needs of current trends within the business
  • Coordinate with departments to ensure services are provided, maintain list of hospitalized clients to confirm that services continue upon discharge.
  • Solid organizational skills including attention to detail and multi-tasking skills.
  • Strong written and verbal communication skills.
  • Proficient in computer use, including internet, word and excel.
  • Provides guidance and counseling to coordinators, office staff, marketing and HR to assist them in continually improving all aspects of home health care

 

Other Duties may include:

  • o Outside travel within service area and occasional overnight travel to trade related conferences, distant start of cares (still within the state of Alabama) and recruitment in those areas as directed by DON

Mileage will be reimbursed in accordance to the Department of Finance in the State of Alabama or a company vehicle will be made available.

  • o During emergencies or natural disasters may be required to be on duty for the duration of the disaster.
  • Requires On-Call responsibilities after hours, nights and weekend, which may alternate with other designated personnel, and will be delegated as the company/nurse responsibility grows.

Administration Duties:

  • Assist in policy formation for the company
  • Assist in Administrative Decision making with particular references to the role, functions, and operations of the agency’s clinical services in the company.
  • Clinical Resource Management in Birmingham and surrounding areas of services in the state of Alabama.

 

Quality Assurance Duties:

  • Conduct monthly/quarterly/ as needed quality and risk assessments meetings, identifying the various areas of concern.
  • Conducts supervisory client/caregiver visits as determined by Director of Nursing.

 

 Job Summary

This Registered Nurse position is responsible to successfully direct and coordinate activities for the franchise in senior living communities. Additional functions may include but not limited to in-home nursing assessments, in-home skilled care, delegation of skilled nursing duties and oversight of skilled nursing care.

Primary Responsibilities:

  • Serves as a liaison with the hospital personnel, physicians, community organizations and other health related service agencies to provide care to the clients.
  • Maintains professional demeanor at all times when interacting with clients and families. Uses tactful, diplomatic communication techniques in potentially sensitive or emotionally charged situations.
  • Conducts pre-admission level of care assessments on potential clients and assures completion of on-going assessments and service plans.
  • Provides case management services to clients and coordinates interdisciplinary service plan meetings with clients and families as care needs change.
  • Investigates and documents client and associate incidents, identifies safety hazards, and institutes corrective action in a timely manner. Shares in the responsibility for providing and maintaining an environment that is conducive to the safety and physical and emotional well being of clients and associates.
  • Assures follow-up with dietary and pharmacy consultant recommendations.
  • Coordinates services with Home Health, Rehab, and Lab, Podiatrist and/or Hospice services as needed.
  • Understands and uses Centers for Disease Control and Prevention (CDC) Standard Precautions, OSHA's Occupational Exposure to Blood borne Pathogens standard, and follows established infection control procedures, hazardous communication, and other safety rules.
  • Operates designated equipment, copy, scan and fax machines and telephone.
  • Monitors care staff proficiency in caregiving. Completes clinical skills competency evaluations.
  • Responds to emergencies in a prompt, calm manner.
  • Monitors e-call system equipment to ensure proper working condition as needed.
  • Holds periodic Wellness meetings.
  • Assists in developing and implementing client marketing plan. Will support and aid the marketing efforts by meeting with prospective clients in a timely manner as needed in retirement communities.
  • Maintains close communication with the Owner/Agency Director regarding client/retirement home resident conditions, changes in condition, problems, potential problems or concerns, incidents, and complaints by residents and/or family members.
  • Acts as ambassador and public relations representative to guests and other visitors.
  • Participates in and supports the client/resident-centered activity program.
  • Demonstrates an appreciation of the heritage, values, and wisdom of the residents and an understanding of the aging process.
  • Collaborates with associates in other departments to assure necessary care and services are provided.
  • Participates in projects or committees as assigned.
  • In the absence of the Owner/Agency Director/DON, has responsibility for overseeing skilled nursing care, skilled tasks, assessments and starting new cases.
  • Performs other duties as requested.

  • Compliance with all policies and procedures of the assigned institution and Right at Home. • Participates in the assessment of patient needs and implementation of patient care required for a patient or group of patients by:
  1. Providing for the emotional, physical and safety needs of the patient or patients.
  2. Observing, documenting and reporting to the appropriate person.
  3. Performing nursing procedures for which his/her preparation has provided the necessary skill and judgment.
  4. Assisting with the rehabilitation of the patient or patients according to the patient care plan.
  • Participates in patient care planning according to the assigned institution’s policies.
  • Performs and/or supervises basic nursing duties that fall into the scope of practice in accordance with the Registered Nurse Practice Act of the State.
  • May receive physician’s orders and transcribe orders, according to the assigned institution’s policies and state laws.
  • Observes and records symptoms, progress and results of medical and nursing interventions.
  • Notifies appropriate personnel of any unusual symptoms, change in condition, etc.
  • Assists in teaching and directing non-professional personnel when required.
  • Make periodic rounds of each assigned clients.
  • May assist with admissions and discharges according to the assigned institution’s policy.
  • Assists physicians with patient examinations and specialized procedures.
  • Participates in patient and/or family teaching and health promotion activities.
  • Safeguards an institution’s or patient’s equipment and supplies.
  • Follows RAH and the assigned institution’s policies regarding infection control including Bloodborne Pathogens.
  • Reports safety hazards and unusual incidents involving patients, visitors, staff or self according to the facility and RAH policies and procedures.
  • In addition to the above, an RN may perform the following, provided he/she has been trained, has demonstrated skill in performing the procedures and the institution’s policies and the State’s Nurse Practice Act allows him/her to perform these procedures:
  1. Medication administration and recording, including chemotherapy and blood administration.
  2. Insertion and maintenance of IV’s.
  3. Insert and irrigate tubes.
  4. Suction endotracheally with or without the benefit of an endotracheal tube.
  5. Extubate patients.
  6. Administer oxygen.
  7. Draw blood for laboratory testing.
  • Updates skills and knowledge by attendance at continuing education courses, inservice classes, etc.
  • Completes other duties as assigned by Supervisor.
  • Informs office and provides documentation of all newly acquired skills, continuing education units, advance education, etc.
  • Certain units require special training skills and proven competency in addition to the usual skills of a RN. Nurses with documentation of the necessary skills may be assigned to the following:
  1. Medical/Surgical Intensive Care
  2. Coronary Care Unit
  3. Dialysis Unit
  4. Emergency Room
  5. Operating or Recovery Room
  6. or any other specialized unit

Education, Experience, Knowledge, Skills, Abilities and Availability

  • Graduation from an accredited school of nursing and 1 year experience within the past 5 years as a RN in an acute facility.
  • Current State license without encumbrances and in good standing as a Registered Nurse by the State Board of Nursing.
  • Knowledge and ability to provide skilled nursing care as defined in the State Nurse Practice Act. Passing score on the RAH Registered Nurse Competency Exam.
  • Current CPR Certification.
  • Use of an insured automobile and a valid driver’s license or access to adequate transportation. • Available for work on weekends and holidays as required.
  • Read, write, speak, and understand English as needed for the job.
  • Annual tuberculin skin test or x-ray and periodic health statement signed by a licensed health examiner and any other health tests or immunizations required by facility.

 

Inservice Training

Assumes responsibility for personal professional development. Participates in inservice education programs as State law mandates.

 

Working Conditions/Environment

Primarily works in private homes and/or facilities, travel required. Shift times and days vary with facility needs.

  • Graduate of accredited school of nursing (preferred)
  • Must be a licensed nurse in state where you work (preferred)
  • Minimum of two years nursing experience (preferred)

  • Dementia care experience (preferred)
  • Must possess a high degree of interpersonal skills and capability of relating to a variety of people and personalities. Considerable initiative, judgment, and leadership skill are also required.
  • Must possess strong organizational and time-management skills.
  • Be in good physical health and free of communicable diseases. Lifting, bending and pushing medication cart about residence is required. Must be able to lift 40 pounds.
  • Completion of drug testing and criminal background check upon hire and upon request of supervisor.
  • Responsible for patient care in accordance with all policies and procedures of the assigned institution and all other applicable RAH procedures manuals, laws and relevant professional standards.
  • Assumes responsibility and accountability for the practice of professional nursing in a manner consistent with RAH policies and the State Nurse Practice Act.

Demonstrates teamwork and effective communication to accomplish patient and agency goals/outcomes.

  • Supervises those to whom nursing functions have been delegated on an ongoing basis and in accordance with regulatory and Agency requirements.

 

 Retains responsibility and authority for determining appropriateness of plan and implements corrective actions when necessary.

Applies knowledge of clinical and administrative policies and practice procedures as outlined in Right at Home policy and procedure manuals.

  • Communicates potential risk management issues appropriately.
  • Reports abuse or unsafe/illegal practice as required by law.
